Client Rights and
Responsibilities
AS A CLIENT YOU HAVE THE RIGHT....
- To be treated with dignity, equality, and
respect.
- To a safe counselling environment, free of
harassment or abuse.
- To be an active participant in determining an
individual plan for your treatment. The therapist will facilitate this
process.
- To be informed clearly and concisely by the
therapist about the available options and recommended strategies.
- To seek a second opinion if you disagree with
the plan.
- To ask your therapist about his/her training
and experience in the issue you are dealing with.
- To know what the therapist has written in
your file.
- To be protected from information disclosure
without a signed consent.
- To confidentiality, unless
- a life is in danger
- a child requires
protection
- a subpoena, warrant
or order is issued and your therapist is called to testify
Staff will not acknowledge you outside the office
unless such is initiated by you.
AS A CLIENT YOU
ARE RESPONSIBLE …..
- To do the work and readings that you have
agreed to for successful completion of your program plan. Your therapist
may discontinue service if there is a lack of follow through.
- To arrive on time, and if there is reason to
re-book, to do so with 24 hours notice.
- To notify the office should your phone number
or address change.
- To ask questions if you do not understand
something during the session.
- To maintain confidentiality in regard to
other people who you encounter seeking counseling services at the office
or in groups.
- To pay the fee for service, to apply for
Employee Assistance Program (EAP) support, or make other financial
arrangements for payment.
